Please print the following forms, sign and bring to tryouts August 24th or August 26th. All signed forms must be turned into Club before a Player can skate.

Everyone needs to complete this form. Returning players will use last season's jerseys and will keep their current jersey#. Invoicing will be sent August 28, 2020 (once the team members have been assigned)

Please register for your 2020 - 2021 USA Hockey Number before completing NVHC 2020 - 2021 Fall Registration.

Safe Sport Training
If your child turns 17 by December 31st of this year they must complete the SafeSport Training by August 20th, 2020 to be put on a roster. This is an AHAI requirement.   Follow this link for SafeSport Program https://www.usahockey.com/safesporttraining
